2011 Teen Tech Week Evaluation

Please complete the following evaluation to relate your experience and outcomes with your 2012 Teen Tech Week grant program. We need your feedback, positive or negative, so that we can report on how the grant funds were used, how programming ideas worked, thoughts for future plans and any impacts already known.

This evaluation needs to be completed along with your Request for Reimbursement (which MUST include copies of invoices/receipts of expenses) no later than April 6, 2012.
