2012 MetroLacrosse Doughtnuts Club Player Waiver

The Stale Doughnuts are Over-30 and Over-40 lacrosse teams that play in the ONELL league. We raise money for MetroLacrosse, a program that uses lacrosse to instill athletic, academic and social enrichment to over 700 Boston area urban youth.

Our practices are tentatively scheduled for Chelsea HS the first Thursday night in May (May 2), immediately following the MetroLacrosse youth team practices.

Season Starts:
Tuesday, June 11 for the Over-40 Blue Stale Doughnuts
Thursday, June 13 for the Over-30 Red Stale Doughnuts

Medical Attention *

I hereby give my consent to MetroLacrosse and other host organizations to provide, through a medical staff of its choice, customary medical/athletic training attention, transportation and emergency medical services as warranted in the course of my participation in MetroLacrosse sanctioned events.

Waiver and Release *

I am fully aware of and appreciate the risks, including the risk of catastrophic injury, paralysis and even death, as well as other damages and losses associated with participation in MetroLacrosse events. I further agree on behalf of myself, my heirs, and personal representatives, that MetroLacrosse, the City of Chelsea, Warrior Lacrosse, Cascade Lacrosse, Commonwealth Lacrosse, the Olde New England Lacrosse League, and any additional sponsors or hosts, along with volunteers, employees, agents, officers, directors of those organizations, and any medical staff and trainers (as specified in paragraph 1 above), shall not be liable for any injury, loss of life, loss of property, or other loss or damage occurring as a result of my participation in the event. Any disputes arising out of my participation in MetroLacrosse activities shall be brought only in Massachusetts court or tribunal to the exclusion of all other forums and shall be governed by the laws of the Commonwealth of Massachusetts.
