Member organizations are required to read, sign and submit a copy of the CONTACT USA "Statement of Values" which can be found within the Agency and Helpline Program Accreditation Manuals on our website (clicking on links will navigate you away from this page and clear any information you have entered in the form, so get those documents before filling out your application!). Please submit this signed document along with a check for membership and/or accreditation.

**Please note that for agencies which are seeking accreditation, all documentation is due six weeks prior to scheduled site visit. Examiners will not make travel arrangements until documentation is received. See CONTACT USA Accreditation Manual and consult with the Accreditation Coordinator for more information.**
