Nomination Guidelines

Delta Dental of Minnesota Foundation announces a leadership award in honor of Ann K. Johnson, former Executive Director of Delta Dental of Minnesota Foundation, and Community Affairs Director of Delta Dental of Minnesota.  Ann dedicated her career to uplifting the lives of others and encouraging those around her to use their talents towards improved oral health for all Minnesotans.


The Ann K. Johnson Minnesota Community Health Spirit Award will be given to an individual in special recognition of their contributions to improving oral and/or overall health in Minnesota, and to celebrate their visionary leadership.




An award of $2,500 will be presented by Delta Dental of Minnesota Foundation. The award is intended to directly award the recipient for their spirited leadership in oral and/or overall health on behalf of underserved communities.


Selection Criteria


Prospective nominees will be considered based on Ann’s leadership qualities:

1.  Inspiring and bringing together people to achieve oral health and/or overall health goals

2.  Leading by example through a lifelong commitment and dedication to communities

3.  Seeking continual growth through listening, learning, and practicing creative leadership

4.  Achieving significant accomplishments for community health, especially oral health

5.  Promoting a positive work experience - working with a smile in all that is done, and having fun in the work you do

6.  Helping others shine and rise to their full potential


Nomination Process


Nominations are invited from throughout Minnesota.  A select group of community leaders that have personal and professional insight into Ann Johnson’s legacy will carefully review all nominations.  Reviewers will interview finalists and make a recommendation for the most compelling candidate to the Delta Dental of Minnesota Foundation Board Chair, who will select the recipient. 


Nomination Procedure


Candidates must be nominated by an organization or by an individual (such as a health care provider, community member and leaders, higher education and government employee, etc.) with whom they are personally or professionally affiliated.  All candidates must have a nomination package submitted online as outlined below.


Nomination packages must include the following items:

·      Curriculum vitae or biographical sketch of nominee

·            Nomination form completed online, providing a narrative description of how the candidate largely meets the selection criteria and why they should be selected (no more than approximately 500 words or 2 pages)

·            Websites, publication citations as appropriate

·            Contact information: Nominator and nominee’s full name, current mailing address, telephone and email address.


Key Dates


Nominations will be accepted online through July 31, 2014. Nominations will be reviewed in August, and awarded in September 2014.




·        · Organizations or groups are not eligible for nomination.

·        · Persons nominated, or actively running for political office are not eligible.

· Persons currently enrolled in a degree program should be able to verify they are in good academic standing.
