Dear Students, Parents, and Legal Guardians:

 

Welcome to Madison Park Development Corporation's 2020 Danette Jones Scholarship Fund, Ralph D. Smith Community Service Scholarship, and Haynes Family Scholarship Fund. These scholarships are intended for full-time high school seniors, progressing normally toward graduation in Spring/Summer of 2020. Our scholarship awards based on a statement made by Ralph D. Smith is for student “who have demonstrated community service in the Madison Park Village and Lower Roxbury areas,” which include and are limited to residences in the 02119, 02120, and 02121 zip code areas.

 

Please answer each question completely and carefully. Please do not leave any blank spaces. Incomplete applications will not be accepted.  If you have questions about completing this application please email Yhinny Matos at ymatos@madison-park.org.

 

In addition to completing this application, all applicants should be prepared to submit all required documentation listed on the application checklist by application deadline, 5:00PM on June 15, 2020.

 

All information provided will be kept confidential and used only for the purposes of making scholarship decisions.

APPLICATION CHECKLIST

    1. Completed Scholarship 2019 Application form

    2. Official High School Transcript

3. Copy of the Federal Free Application for Federal Student Aid (FAFSA) and the responding Student Aid Report (SAR)

    4. Copy of Acceptance Letter from the university, college, or vocational training program you plan to attend.

     

    5. Copy of Financial Aid award letter from the university or college you plan to attend, if available.  If not yet available, please indicate when you expect the information to be available.  The Scholarship Committee cannot make a decision regarding your scholarship without this information.

     

    6. Letter from your parent(s) or guardian(s) explaining how your college expenses will be met.

     

    7. One letter of recommendation providing information on your academic ability, community service record, family background and potential for success.  The letter is required from one of  the following sources:
      • Current School Official (Principal, Guidance Counselor, or Teacher)
      • Community Leader (Minister, Civic Leader, or Employer)

    Each letter should be submitted with the application in a sealed envelope and the individual providing
    the     recommendation must put their signature across the seal of the envelope.   

     

    8. Essays - You are required to submit one essay.  Essays must be typed, double-spaced with one inch margins.  The suggested font is Times New Roman and the font size 12.  The essay must be a minimum of 500 words and must be on one of the following topics:

     - Describe the role that community service has played in your life and in your education.

     - Write a brief autobiography and describe key incidents in your life that have made you the person that you are.

    - Discuss your professional and personal goals and the role that community service will play in your  future.

    9. A copy for your records.  Please be sure to make a copy of the completed scholarship packet for your records.
    10. Provide a photograph of yourself.

Applicants are required to be a U.S. citizen or eligible non-citizen.  We consider you to be an eligible non-citizen if you are:
- a U.S. permanent resident with a Permanent Resident Card (I-551)
- a conditional permanent resident (I-551C)
- a holder of an Arrival-Departure (I-94) from the Department of Homeland Security showing any of the following designations: Refugee, Aslyum-Granted, Parolee (I-94) (confirms paroled for a minimum of one year and status has not expired); or

For the Jones and Haynes College scholarship applicants,you are required to submit one essay. The essay must be typed, double-spaced with one inch margins.  The suggested font is Times New Roman and the font size 12.  The essay of a minimum of 500 words must be on one of the following topics:

B. Write a brief autobiography and describe key incidents in your life that have made you the person that you are.

FOR HAYNES FAMILY SCHOLARSHIP FUND APPLICANTS WHO ARE APPLYING FOR A VOCATIONAL, CERTIFICATE OR TRADE PROGRAM ONLY:

A. What do you expect to gain from your educational experience.

B. Write a brief autobiography and describe key incidents in your life that have made you the person you are.
