This form is for individuals or organisations running activities to fundraise for Multiple Sclerosis Limited who do not require the creation of a personalised online donation page. By completing this form and agreeing to our fundraising terms and conditions we will be able to generate your official Authority To Fundraise documentation and provide you with the appropriate print materials or cash donation supplies to support your fundraising activity.

- If you are wanting to register a fundraising event and have an MS online fundraising page created for the event please register via www.doitforms.org.au.

- If you are wanting to register to host an MS Donation Tin at your premises and are not planning to coordinate any additional fundraising activities/events please register here.

Once you have submitted this form a member of the team will make contact with you to discuss your fundraising goals, confirm your event is officially registered and explore what personalised fundraising assistance we can offer you to make sure you have a fantastic time fundraising for MS!

I acknowledge having read and I agree to abide by the fundraising rules and Guidelines of Multiple Sclerosis Limited and indemnify Multiple Sclerosis Limited from and against any claim for injuries or damage arising at or from the project/event that is the subject of this application.

I understand and agree that all publicity for the proposed event must be approved by Multiple Sclerosis Limited prior to being released and printed.
