Complete applications are due to the Foundation no later than 5:00 PM on March 15. This form must be completed and supporting documents must be uploaded or postmarked (if mailed via US Mail) by the deadline. Academic Information

List the name of the high school you attend currently.

List address of your high school.

Provide your Grade Point Average as of the end of the Fall term of your Senior year. (This number will be verified against the transcript supplied by your school.)

Provide your Class Rank as of the end of the Fall term of your Senior year. (This number will be verified against the transcript supplied by your school.)

Provide the total number of students as of the end of the Fall term of your Senior year. (This number will be verified against the transcript supplied by your school.)

List the month and year in which you will graduate.
Educational Plans
Provide information for the college, university, technical school or other educational institution you plan to attend.

List the name of the educational institution you plan to attend.

List the name of your degree program or field in which you plan to study. You may list that you are undecided if you have not yet made a decision.

i.e., Bachelor of Science, Bachelor of Arts, Bachelor of Fine Arts

List the month and year you will begin your studies.

List the month and year you plan to graduate.
Supporting Documentation
The following supporting documents must be submitted to the Foundation in order for your application to be considered. Incomplete applications will not be considered.

You are responsible for having a copy of your official high school transcript submitted to the Foundation by the appropriate school personnel. The transcript should cover the first three and one half years of high school through the end of the Fall term of your Senior year. (Unofficial transcripts will not be accepted.) It should include a list of courses you have taken and the grade received in each course, your class rank and SAT® and/or ACT® scores.
Standardized Test Scores (OPTIONAL)
You are encouraged, but not required, to submit your official SAT® and ACT® scores. If you would like to submit these scores, contact your Guidance Counselor to see if scores are included on your official transcript. If scores are not included on your transcript, send the Foundation a copy of your score notification letter from The College Board.

Essay

The essay should describe your career objectives and the contributions which you expect to make to the public good while achieving those objectives. It may also describe your academic accomplishments and participation in extracurricular activities, However, it should not be redundant to the lists of scholastic honors, extracurricular activities and memberships submitted in previous sections of this application.

Please list three references who are not family members or employees of Greenleaf Corporation or any of its affiliates.
