The candidate or ministry staff person below, has requested that you provide us with the information below to help us evaluate their qualification for ministry leadership and then to stand with them if their qualification is ever questioned. It is not the purpose of this questionnaire to know all the details of the divorce. We seek to gather relevant information needed to make an informed decision concerning their potential status with A.C.T Intl.
