In order
for your Certification of Professional Achievement to be conferred, you must
submit this application form before the deadline listed on the Certification Completion page. If you missed the deadline, you may still submit
this form and your application will be reviewed during the next review period.
Be
sure to provide your name as you wish it to appear on the Certification. Also,
remember to provide a full mailing address that will remain current for a few
months after the conferral date. If you do not provide a valid mailing address,
this will cause delays in processing.
If you are
completing a Certificate and not a
Certification of Professional Achievement, you must submit the Application for
Certificate form, which is found on the Certification Completion page.
Applications
are reviewed three times per year. Certifications will be printed and mailed
approximately four weeks following the October, February, and May conferral
dates (listed on the Certification Completion page). If you have any questions,
