Admissions Petition

Please submit this petition to be considered for readmission to GWSPH graduate programs. If you were suspended from the university, you must also submit an application via SOPHAS Express. You must complete at least one semester of GW graduate coursework with grades to be eligible for this petition. 

Please fill out the form below and attach your GW transcripts. Your petition cannot be processed without your GW transcript. Requests must be made at least one month prior to the semester you wish to matriculate as petitions generally take 4 or more weeks to process.
