Return from a Leave of Absence

Students returning from an approved leave of absence should submit this form preferably six weeks before the start of the semester in which they wish to return. Students returning from a medical leave must submit to the Office of Student Affairs a doctor's note approving their return; they must also contact a Columbia health provider who will evaluate their readiness to return.

FOR STUDENTS ON MEDICAL LEAVE: Please submit a letter from your health care provider that confirms that you are capable of returning to graduate study and details any recommended qualifications. You will not be able to register until you do so. Letters should be sent to:

Please save the email confirmation you will receive after you submit this form as documentation of your having submitted this request.

Once your request has been processed, you will receive an official communication from the Office of Student Affairs regarding the remaining steps necessary for your return.
