This form is used to establish the condition of the unit upon move-in and move-out. When moving in, this form must be submitted within 48 hours of your move-in date. 

Please go around the apartment and use this form to comment on the current status of the unit. You can use phrases like "ok", "good", "scuffs on wall", "paint chipping", or other ways to describe the conditions. The more detailed, the better. 

For the bedrooms, please describe the location of the bedroom and list who's bedroom it is under "Other comments"; for example: "Emily's bedroom, left of full bathroom".
