Employee Recognition and Grateful Families Donation Form
Has someone made you or one of your loved ones feel special? Or gone out of their way to help you? 
Here is where you can recognize this person for their service!

Recognized employees will be honored at their campus at their next employee recognition meeting. Donations made in memory of an employee will be distributed to their family. Donations made to recognize active employees will be distributed to the Trilogy Foundation to benefit Emergency Assistance and Education Assistance programs.  
Acknowledgement will be sent to individual or person designated if provided in form. 

Each of our campuses are supported by the Trilogy Foundation, a 501c3 non-profit organization dedicated to serving our Employees, Residents, and Communities.  
All Gifts to the Trilogy Foundation will be used to support:
  • Our employee Emergency Assistance program, designed to benefit employees in times of personal and family crisis. 
  • The Education Assistance program, which has awarded more than $4 million to help with our employee's educational endeavors. 
  • Special projects, outlined by our campus administration, to continue to enhance the resident experience.

PTO Donations - Hourly Employees Only

Due to IRS policies and tax ramifications your PTO donation can not be designated to a specific person through the Trilogy Foundation. However, you can cash out your PTO and donate it directly.

The PTO Cash Out Form should be taken to your campus AP/Payroll Coordinator of Business Office Manager.
