This Annual Report is to be submitted in order to notify the Air District that all of the electric vehicle charging stations comply with the Program Requirements. Each Annual Report shall cover a 12-month period (from January 1 to December 31). Please note, the first Annual Report may not cover a full 12 months.  

Due Date: Every March 1, after the Interim Status Report has been submitted, until the end of Project Useful Life.

If the project includes more than one charging facility, then this section must be completed for each distinct facility. To add another charging facility, click on "Add Another Facility" on the bottom of the section. 

Refer to Attachment B of your contract for the Facility ID.

For each month, provide the amount of total electricity dispensed at the facility in kilowatt-hours (kWh). This information should cover a 12-month period from January 1 - December 31 of the previous year.

Program. Any misstatements or omissions of material facts may result in cancellation of any monies awarded.
