Returning Students Application Form

Important:
  • This form is to be used by any previously enrolled student, who was previously academically dismissed OR who has not been continuously enrolled in classes at OCC for the past two years. 
  • This form may not be used for Health Profession programs (i.e. Nursing, Physical Therapist Assistant, Surgical Technology).
  • An official high school transcript or GED is required for all degreeā€seeking students.


Individuals wishing to apply for admission to Onondaga Community College for the FIRST time, should follow the steps for new applicants.
