BYA VOLUNTEER/MENTOR AGREEMENT, LIABILITY WAIVER AND REFERENCE RELEASE:

I agree to serve as a volunteer/mentor and to perform my volunteer/mentor duties to the best of my ability, to adhere to BYA rules and regulations including documentation requirements and confidentiality of BYA and program information, to attend volunteer/mentor trainings and meetings, to meet time and duty commitments, or to provide notice so that alternative arrangements can be made.

I release BYA from all liability. I understand that I am covered under BYA’s general liability insurance coverage.

I hereby authorize BYA to verify all information contained on my volunteer/mentor application with former employees, references or appropriate personnel or resources. I further authorize that any personnel at the listed places of employment or reference may disclose any and all information regarding my work history, personal characteristics or other importance to BYA. I also authorize BYA to conduct a criminal check of my background. Furthermore, I waive the right to sue the aforementioned references for releasing such requested information. I understand this authorization policy and by clicking 'Submit' I agree to the release and verification of the aforementioned information.
