If paying by credit card, please click here! The payment form is separate from the registration form, so you will still need to return to this form and submit it to enroll your child. 

Your child(ren) are NOT considered enrolled until full payment is received. Cancellations can be fully refunded up to a week prior to the class. Cancellations within a week of the class will not be refunded.
Liability Wiaver
I certify that the child I represent is healthy, willing and able to physically participate in the activities of Cooking Classes. I understand that my child will be working with food and cooking implements under the guidance of an instructor. I also understand that they are participating at their own risk. In order to be safe, they agree to follow the instructions of the staff.
Behavior Standards
It is Community Harvest Project’s goal to provide a physically and emotionally safe place for staff, volunteers, and program participants. We ask program participants to respect themselves, others and our environment, by following the rules and instructions given during activities and using appropriate language. Inappropriate behavior will be addressed immediately. Consequences may include participants being restricted from activities, sent home, and/or suspended from future activities without a refund. Open communication is encouraged between staff and parents. Community Harvest Project has a very strict “hands off policy” any fighting or other unsafe behavior may result in immediate dismissal from programs.
Photo Permission
Help share our story! One of the best ways to share what’s happening here at Community Harvest Project is to show our volunteers and staff in the fields, doing what we do best together. We typically use photographs in our newsletter, some brochures and on our website.
