Please complete this section, even if you have volunteered for Alaska Health Fair for years. We ask that volunteers and Lead Phlebotomist fill out this special section every other season.

Standards Agreement

The Volunteer warrants that:

  1.  I am competent to perform the volunteer duties selected. I will do so to the best of my ability and in a safe and reasonable manner.  I am not legally prohibited from providing any of the services for which I have volunteered.  If requested, I will provide Alaska Health Fair, Inc. with any additional information necessary to verify that I am qualified for this position.
  2. I am participating solely on a volunteer basis and do not expect any compensation for services rendered.
  3.  I agree to follow the current written protocol for my station, and any general policies or procedures for the event. I will follow the directions given to me by the Site Coordinator or other Alaska Health Fair, Inc. representative.
  4. I will respect diversity and refrain from making offensive comments about race, gender, age, religion, physical ability or sexual orientation. I will treat other volunteers, participants, etc. with respect and will use appropriate language. I understand that sexual harassment will not be tolerated. I understand that all health fairs are drug free and smoke free environments. I agree to wear attire that is appropriate for the volunteer duties selected, and which projects professionalism and facilitates a pleasant environment for event attendees (clean, ironed, and worn correctly, etc.).
  5.  I understand that all participant information is confidential. I agree to protect the confidentiality of participants and will not discuss their personal information, medical condition, or any other information with any other person. I also agree not to use, copy or retain any participant information except in accordance with the protocols for my volunteer position.
  6. I understand that the Alaska Health Fair, Inc. reserves the right to reject my application at any time, should its representatives determine that my participation in the event may be detrimental to myself or the participants.
  7.  I am participating at my own risk and will hold the Alaska Health Fair, Inc., its agents, officers, and employees, harmless from any liability whatsoever, for any injuries, damages, claims, and losses I may incur as a result of my participation in an Alaska Health Fair event.  I will indemnify Alaska Health Fair, Inc. from any and all liability whatsoever, for any injuries, damages, claims and losses arising out of my involvement as a volunteer.
  8. In the event of an emergency, I grant Alaska Health Fair, Inc. permission to authorize my emergency medical care and treatment.

Alaska Health Fair, Inc. agrees to:

  1. Provide training, information and assistance for the volunteer to be able to meet the responsibilities of their position.
  2. Provide a written protocol for each volunteer position, along with any general policies or procedures that apply to the event generally.
  3. Respect the skills, dignity, and individual needs of the volunteer, and do our best to adjust to individual requirements.
  4. Be receptive to any comments from the volunteer regarding the way in which we might better accomplish our respective tasks.
  5. Treat the volunteer as an equal partner with agency staff and to recognize that we are jointly responsible for the accomplishment of our mission "to be a vehicle for the presentation of health and education and medical screenings across the state of Alaska."
  6. Adhere to a non-discriminatory policy. Volunteers will not be treated differently due to race, gender, age, religion, physical ability or sexual orientation.

We will email you a copy of this form for your records. Approximately one week before health fair you will receive a confirmation from AHF with description of duties and additional information.
