If you are submitting information on behalf of educators, you'll receive an email with further instructions within the next 24 hours on where to upload this form. In order to streamline the process and submit on behalf of educators, please download the Teacher Worksheet and enter ALL required cells. Please save as a CSV file. If submitted as a PDF, the form will not be reviewed. Only information on the first tab of your document will be saved and reviewed. If cells are left blank, the educators will not be considered for recognition. DO not cut and paste information from another document.
Please attach three pieces of documentation, such as an outline of the school PD, a sample of student work, and/or a flyer from a Connecting Families event, to this application.
