NOTE: It is important that you answer all questions as thoroughly and specifically as possible. Your responses will assist us in placing you in a home stay but this form will not be shared with the host families. Please keep in mind we may not be able to meet all of your expectations. Certain requests are given higher priority than others, e.g., students who are allergic to animals will be given a non-pet household. Placements will be based on “best possible fit”.

Most students are housed with host families located between Tallories and Annecy. Annecy is a longer commute and is a small, touristic city. Talloires is a small-town setting with a shorter commute to and from class. Please note a majority of student will have to take public transportation to and from Talloires.

Please read this form carefully. By signing this form you are agreeing to the terms and conditions described below.

  1. I, the undersigned, a participant in Tufts in Annecy (hereafter “the Program”), hereby, on behalf of himself or herself, heirs, executors, administrators, assigns, agents and representatives, and anyone else claiming by and through him or her (collectively the "undersigned"), waives and releases Tufts University, its former and current trustees, officers, directors, employees and agents (collectively "Tufts") from and against all claims or causes of action which the undersigned may have, now or in the future, relating to any injury, loss, damage, accident, delay or expense generality of the foregoing, but not limited to the use of any vehicle, the occurrence of strikes, war, governmental restrictions or regulations, or the acts or omissions of any water carrier,airline, railroad, bus company, taxi service, hotel, restaurant, school, university, or any other firm, agency, company, host family,or individual. I hereby agree to exonerate, indemnify and hold Tufts harmless from and against any and all obligations or liabilities for which I may become liable as the result of damage or injury to the person or property of others while participating in the Program. I expressly acknowledge and agree that Tufts shall not be responsible for any injury or loss whatsoever suffered by me during periods of independent travel (which I understand are unsupervised) or during any absence from activities supervised by Tufts.

  1. I have weighed the dangers inherent in foreign travel and study, the risks presented to my own health and well-being, and my personal desire to further my educational experiences by traveling and studying in a foreign country. I have concluded that the risks are acceptable and are outweighed by my desire to participate in the Program.

  1. During my participation in the Program, I hereby grant Tufts full authority to take whatever actions it may consider is warranted under the circumstances regarding the protection of my health and safety. I hereby release each of them from any liability for any such decisions or actions as may be taken by them in connection therewith. The authority granted in the preceding sentence shall include the right to place me, at my own expense and without any further consent, in a hospital, within or outside the United States, for medical services and treatment, or if no hospital is readily accessible, to place me in the hands of a local medical doctor for treatment. If it is deemed necessary or desirable by Tufts, its employees or agents, I authorize them to transport me back to the United States, by commercial airline or otherwise, at my own expense, for medical treatment.

  1. I understand that every attempt has been made to provide a suitable, safe living environment for me the duration of the Program. I understand that the host family accommodations arranged by the office of the European Center will include either a single or double (with another Program participant) room arrangement, and three meals a day; a packed lunch shall be provided by my host family for consumption on weekdays. I understand that changing host families once the Program has begun may incur a change fee. I understand that the room and board arrangements between Tufts and my French host are valid from the Program's stated arrival date until the Program’s stated departure date. I understand that I am obliged to leave the host family accommodation on the final day of the Program. Tufts shall not be responsible for living arrangements made by me after the conclusion of the Program.

  1. In the event that Tufts shall advance or loan monies to me or shall incur expenses on my behalf while I am a participant in the Program, I hereby agree to repay any such advances, loans or expenses immediately upon my return to the United States.

  1. I hereby agree to comply with all rules, standards, and instructions set forth by the Tufts University Code of Conduct.

  1. The Tufts University Code of Conduct applies to all matriculated students regardless of location, time of year or leave status. I understand that due to the location and immersive nature of the Tufts in Annecy Program, there is a high expectation for students to be productive, positive, contributing members of the Tufts Community. I agree that Tufts shall have the right to enforce appropriate standards of conduct and that Tufts may, at any time, terminate my participation in the Program in the event of any failure to abide by any such standard of conduct. If my participation in the Program is so terminated, I agree to leave the lodging of my host. Subsequent food, board, and transportation costs will be at my own expense. Tufts shall be under no obligation to refund any portion of the fees and other charges which have already been paid or afford me any credit against fees and charges which have accrued but have not then been paid.

  1. When participating in group tours or other activities arranged by Tufts, I agree to accept the will of the majority whenever a matter of choice is presented to the group. I will also accept in good faith the instructions and suggestions of Tufts or their agents in all matters relating to the Program including the personal conduct of Program participants.

  1. I understand that, from time to time, Tufts’ publicity materials may include statements, photographs and/or video footage of Program participants, and I hereby consent to such use of my comments and photographic likeness.

  1. I hereby acknowledge that Tufts reserves the right to alter Programs, itineraries, fees, and courses. Tufts reserves the right to cancel any Programs due to insufficient enrollment, in which case all Program fees are refundable. Tufts University European center is not responsible for nonrefundable air ticket costs.

  1. I understand that the $500 deposit is non-refundable. In the event that I withdraw from the Program 30 or more days before the Program begins, I will forfeit my deposit. If I withdraw less than 30 days before the Program starts, I will receive a refund of one-half of the total Program fee. I agree that, if I leave the Program after the first day of the Program, there will be no refund of any fees or charges already paid or any credit for fees and charges accrued but not paid, except under extraordinary circumstances determined on an individual case basis. Requests for withdrawals must be submitted in writing to the Tufts University European Center.

  1. All references in the Participant Agreement Form to Tufts and their employees or agents shall include the officers, directors, staff members, mountain guides, chaperones, group leaders, employees, agents and affiliates of Tufts, as the case may be.

  2. I hereby acknowledge that I have read the terms and conditions set forth in this Agreement and Release, and I understand that this agreement constitutes the entire agreement between Tufts and myself.

  1. This Agreement shall be construed in accordance with the laws of the Commonwealth of Massachusetts.

TRANSCRIPTSIf you have a copy of your transcript, please attach it below. An unofficial SIS printout will be accepted. Freshman registrations will be held as pending until the first semester transcript is submitted to the European Center, no later than the first day of spring semester classes. Transcripts may also be emailed to france@tufts.edu or dropped off in person.

PASSPORT: If you have a copy of your passport, please attach it below. Students may also email, france@tufts.edu or drop off a copy of their passport if they are in the process of applying for one. We should receive this no later than March 29, 2019.

If you are not a Tufts University student, please have your academic adviser and an additional reference submit a Recommendation Form online (2 recommendations required) and be have your university transcript emailed to france@tufts.edu or sent to the address below:
