Project Information

Separate project codes per day require separate form submissions!

For disaster response: When you are traveling from your shop location to the deployment site, record the travel time to the legislative district of your home shop; i.e. do not record each state you drive through. Once you have reached the deployment site, report the legislative district for that location.

Activity Reporting

Must be provided using the unit of measure specified in CAPS at the end of the activity description (i.e. rain gardens in SQ FEET, exotic/invasives mgt in ACRES, non-motorized trail in FEET, etc.)

Numeric value only! Total crew hours spent on task this day, i.e. enter 50 if a 5-person crew spent a 10-hour day on this activity.

Total number of volunteer hours, i.e. if 2 volunteers worked for 3 hours each, enter 6.
Reports & Stories

Additional notes about invasives removed, tree/shrub types planted, youth behavior, project host interactions, etc.

If you'd like a copy of this report emailed to someone, enter the recipient email address here.

If you select "yes" here, you can upload two additional photos below. Photos are best when everyone is wearing proper PPE and working safely! They cannot be used on Corps social media otherwise.

Ideas: Upload before/after photos from your project this week. Describe the project. How did it impact the land/community? How did the project impact you/the crew? Did you learn new skills or overcome any challenges?

i.e. merit report, close call form, youth behavior contract, etc.
