You must arrange for your campus liaison to complete this form and return it to The Washington Center Enrollment Services, if your campus needs your grades earlier than four weeks after the end of the term. When this form is received, your TWC Lead Instructor and your course instructor will be notified.
Your Lead Instructor will notify you of the date your grades are due to your school and the date by which you need to submit your materials to him or her. You must follow up with your professor and make arrangements to complete assignments early so that your grade can be submitted on time.
It is the INTERN’S RESPONSIBILITY to ensure that:
- He/she completes the assignments necessary in time for his/her grades to be sent to his/her college/university.
- All outstanding balances for which the intern is responsible have been paid.
- This form is sent by the Liaison to The Washington Center Enrollment Services by the deadline.
Evaluations will be transmitted electronically.
