I hereby give permission to Springboard Collaborative, and their agents, to videotape, photograph, and record in any medium my appearance, name, likeness and voice (the “Recordings”). I further grant Springboard Collaborative the right, without restriction, to edit, share, use and reuse, and allow others to use the Recordings in print, on the internet, and in all other forms of media for the purposes of program delivery, organizational promotion, professional development, and general advancement of Springboard Collaborative’s mission. I hereby consent to such use. I hereby release Springboard Collaborative, their employees, their agents, any who use Recordings from all claims, damages, costs and expenses, and liabilities whatsoever which I now have or may hereafter have by reason of any use thereof. I understand that the provisions of this release are legally binding.
