- Required - Copy of the most recent organization Annual Profit and Loss Statement or Annual Audited Financial Report (school districts and colleges please provide adult education program budget only.)
- Required - Current Operating Budget
- Required for non-profits only - Copy of IRS tax-exempt determination letter
- Optional - Other supporting materials (i.e. letters of support, newspaper articles, etc.) Must be limited to two pages.
