This is the Encore Fellowships Work Host Registration and Organization Description form. Once you have submitted this form, you will receive an email from us with a link to add a Fellowship Position Description. You may submit the position form multiple times if you wish to submit more than one position for consideration in the program. Once you have submitted one or more Fellowship Position Descriptions, your application will be complete.


You may save a partially-completed form and finish it later. When you are satisfied with all your responses, click the "REVIEW" button at the bottom of the form. When you click "REVIEW" you will be shown a confirmation page where you may review and edit your responses. You will also have a "PRINT" option at the bottom of the page. Click the "CONFIRM" button to submit your application. You will no longer be able to edit, and your information will be routed to the appropriate Encore Fellows programs for their consideration.

Instructions for Completing Your Application
  1. Click the "REVIEW" button at the bottom of the application.
  2. After clicking the "REVIEW" button you can make edits and finalize your responses.
  3. Use the "PRINT" option at the bottom of the page, if you'd like a copy of your responses.
  4. Click the "CONFIRM" button to submit your application. You will no longer be able to edit your application.
  5. Check your email for the link to the Fellowship Position Description form.
  6. A position form must be completed for your application to be considered.

  • Reference and/or background checks of Encore Fellows are the responsibility of the host organization, consistent with the organization's standard employment practices.
