In consideration for being allowed to participate in The Philanthropy Workshop Program ("TPW”) and the TPW Journey to Canada in September 2018 (the "Trip"), I acknowledge that there are risks associated with the Trip.
By my signature below, I acknowledge that my participation in the Trip may expose me to significant risks, including, but not limited to, crime, terrorism, war, the hazards of travel by airplane and other vehicles, serious bodily injury or death, property damage and other risks. I recognize that TPW cannot guarantee my safety or that of others traveling with me. I also understand that TPW accepts no responsibility for delays, loss or injury caused to others or me prior to departure, while traveling or while abroad, and shall not be responsible to any person for any of my acts or omissions. I assume full responsibility for all risks associated with my participation (including risks to any others I have traveling with me) on the Trip and I agree to release and hold harmless TPW, all of its programs and its directors, officers, employees and agents from any claims, demands, causes of action or liabilities that I may have, including, but not limited to, illness, bodily injury, imprisonment, death and loss of personal property, resulting from or in any way connected with my participation in the Trip.
I also acknowledge that medical coverage provided by U.S. companies does not necessarily cover travel and it may be necessary for me to supplement coverage I have; that neither TPW is responsible for obtaining any health, accident, disability or other insurance for my benefit; and that, to the extent not covered by my insurance, I am responsible for all charges associated with any medical emergency or medical treatment I might have during the Trip or in connection with the Trip.
By signing below, I affirm that I am at least 18 years of age, have read and understood the terms of this Release Form; am aware of my right to consult with an adviser or attorney of my choice regarding it; and am signing it voluntarily and with the intent to be bound by it.
