Gifts from Home Agreement

ELIGIBILITY:  Service members who are currently deployed in support of the Global War on Terror. Must have at least three months left of deployment.


Submitting an application is consent to all terms and protocols.  I understand that by submitting this application, I agree to submit to an in-person or phone interview at Freedom Alliance’s sole discretion.  I also acknowledge that submission of an Application for Consideration does not guarantee selection.  I understand that by submitting this application, I agree to notify Freedom Alliance of any changes to the above-listed information.


Freedom Alliance is committed to providing an environment that is free from discrimination and conduct that can be considered harassing, coercive, or disruptive, including sexual harassment.

 

I hereby release, discharge and agree to save harmless Freedom Alliance, its legal representatives or assigns, and all persons functioning under its permission or authority, or those for whom he/she is functioning, from any liability by virtue of any use consistent with the purpose set forth herein.  For consideration that I acknowledge, I grant to Freedom Alliance the unlimited right to use my image, name, likeness, and information in connection with the program, which may be included in marketing materials (including print, websites, or other electronic forms of media made available nationwide).  In exchange for the intangible value I will gain by being featured in these materials, and other good and valuable consideration, I give Freedom Alliance permission for such and publicity for such purposes. 


To the fullest extent permitted by applicable law, I hereby irrevocably waive all legal and equitable rights relating to all liabilities, claims, demands, actions, suits, damages, and expenses, whether sounding in tort, contract, or any other legal theory, now known or hereafter known in any jurisdiction, arising directly or indirectly from my participation in or application to the program (collectively, “Claims”).  I covenant not to make or bring any such Claims against Freedom Alliance, its legal representatives, or assigns, and forever release and discharge Freedom Alliance from liability under such Claims. I understand that I am not entitled to financial compensation from Freedom Alliance or any of its legal representatives or assigns as a result of submitting an application or participation in the program. 

Heroes to Homeowners Agreement

Freedom Alliance helps wounded heroes achieve the American Dream of home ownership. We provide mortgage-free homes to combat wounded troops. For those who have sacrificed so much, it's a place of hope and promise and an opportunity to begin anew.


The following is a series of Protocols relating to the Heroes to Homeowners program:


Goal of Program


The Heroes to Homeowners program, run by Freedom Alliance, is aimed at helping certain U.S. Service Members (“Candidates”) to become homeowners and enjoy the benefits of home ownership.


Eligibility for Program


1. Candidates are service members wounded by enemy action post 9/11/2001.


2. Candidates must be honorably discharged from the military.


3. Candidates must have 80% or higher disability rating from the VA.


4. Candidates must complete the application for consideration and provide all requested documentation.


5. Candidates must be willing to relocate to accept the available home selected.


6. Candidates may not currently own a home.


Application for Consideration


Candidates must provide complete and honest responses to requests found in the Application for Consideration.


Proper documentation provided in support of the Application for Consideration includes those documents necessary to confirm military service, medical history, injuries, and family status and family history, including DD-214, disability rating from Veterans Administration, or other documents requested.


Among other things, completion of the Application for Consideration authorizes Heroes to Homeowners to complete a credit history, criminal background check, social media checks, or other background checks.


Candidates will meet with member(s) of the Heroes to Homeowners program committee to review the application and documentation if requested.  Candidates will be interviewed by Freedom Alliance, or its authorized representative, for consideration by the program.


Completion of the Application for Consideration does not guarantee acceptance to the Heroes to Homeowners program.  Acceptance into the Heroes to Homeowners program does not guarantee home ownership through the program.  Acceptance merely means that the service member has been pre-screened and approved to enter the Heroes to Homeowners program and be considered to receive a home, should one become available and other criteria be met.

Freedom Alliance will work with banks, lending institutions, and other groups to find a home suitable to the needs of the Candidate, but cannot guarantee that a suitable match will be made or that the Candidate will otherwise be selected.


Two-Year Lease of Residence


If a Candidate is accepted into the Heroes to Homeowners program, Freedom Alliance shall lease a property to the Candidate for up to a two-year period.


The property must be the Candidate’s primary residence.  The Candidate shall not assign, sub-lease, or permit others not approved by Freedom Alliance to reside in the property.


During the lease period, the Candidate shall pay rent of $1 per month (payable in one lump sum at the beginning of the lease period).


Candidate’s lease of the property is premised on evaluating the Candidate for potential home ownership.  Thus, Candidate shall be responsible for all maintenance and repair of property, in addition to all fees, insurance, costs, taxes, and utilities related to property.  Candidate shall be responsible for obtaining and paying for renter’s insurance.  Freedom Alliance shall be responsible for obtaining and paying for property or other insurance.


Prior to accepting lease of property, Candidate shall be entitled to a walk-through, inspection, or certification of property condition.


Representative of the Heroes to Homeowners program shall be entitled to perform inspections of the property.  Refusal to allow inspection of the property and/or the results of inspection will be a factor for considering whether to award full deed of the property to the Candidate.


During the lease period, the Candidate shall participate in a counseling program (whether of a family, financial, or other nature) if deemed necessary by Freedom Alliance.


The Candidate shall not commit any act that indicates dishonesty or moral turpitude or that otherwise could materially injure the reputation of Freedom Alliance or the Heroes to Homeowners program.


Evaluation Following the Lease Period


When all terms and conditions are successfully met during the two-year lease term, Freedom Alliance will consider whether to award the full deed of the property to the Candidate.  At that time, the Candidate is eligible to be submitted for consideration for full ownership of the property.


Factors for consideration include, but are not limited to, the following:


§     Candidate’s compliance with terms of the lease agreement

§     Candidate’s criminal or arrest record (both pre- and post-occupancy)

§     Candidate’s ability to remain drug-free (both pre- and post-occupancy)

§     Candidate’s refrain from overuse of alcohol

§     Candidate’s financial stability

§     Candidate’s ability to maintain property

§     Candidate’s ability to serve as an ambassador for or representative of the Heroes to Homeowners program


If the obligations, terms, and conditions of the two-year Lease Period are not met, Freedom Alliance may extend the term of the lease with mutually agreed terms and conditions or may terminate the lease and require occupant to vacate the home within 30 days.


Terms of Transfer of Deed


If selected to receive the full deed of the property, Candidate shall accept all legal and financial obligations associated with the property.  Candidate shall continue to occupy the property as her or his primary residence.


If selected to receive the full deed of the property, Candidate shall then maintain the property as a primary residence for no less than three years from the date of the deed.  Should Candidate violate this covenant, the property shall revert to Freedom Alliance.


At the end of the three-year term after the award of the deed, if obligations, terms, and conditions are met, the covenant shall expire and Candidate shall own the property free of all obligations to or interest by Freedom Alliance.

Home Repair (R.A.M.P.) Agreement

Goal of Program


The program, run by Freedom Alliance partnering with U.S. Bank, helps certain U.S. Service Members and/or widow(er) of KIA (“Candidates”) to live in the primary residence they own with said property being a suitable and safe dwelling.


Eligibility for Program


1. Candidates are service members who have deployed post 9/11/2001 in support of the Global War on Terror.


2. Candidates must be honorably discharged or medically retired from the military.


3. For Gold Star Families – Candidate must be the widow(er) of a service member KIA post 9/11/2001 in support of the Global War on Terror.


4. Candidates must complete the application for consideration and provide all requested documentation.


Application for Consideration


Candidates must provide complete and honest responses to requests found in the Application for Consideration.  If the information provided to Freedom Alliance changes or is no longer accurate, Candidates must provide updated information.


Proper documentation provided in support of the Application for Consideration includes those documents necessary to confirm military service, documents relating to the candidate’s primary residence, and other documents as requested.


Among other things, completion of the Application for Consideration authorizes Freedom Alliance to complete a credit history, criminal background check, social media checks, and other background checks. Candidates will be interviewed by Freedom Alliance or an authorized representative for consideration for the program.


Candidates must respond accurately and completely to reasonable requests for information from Freedom Alliance relating to the program.


Completion of the Application for Consideration does not guarantee acceptance to the program.  Acceptance into the program does not guarantee approval for repairs.  Freedom Alliance will make decisions based on need and resources available.

Driven to Serve Agreement

"Driven to Serve" is a program of Freedom Alliance partnered with U.S. Bank.


The following is a series of Protocols relating to the “Driven to Serve” program.


Goal of Program


The “Driven to Serve” program, run by Freedom Alliance, is aimed at helping certain U.S. Service Members and/or widow(er) of KIA (“Candidates”), and Scholarship Fund recipients with owning a vehicle in order to assist them in transportation to medical appointments, for use in employment and finding employment, and improving quality of life while providing a cost-free means of transportation.


Eligibility for Program


1. Candidates are service members who deployed post 9/11/2001 in support of the Global War on Terror.


2. For Gold Star Families – Candidate must be the widow(er) of a service member KIA post 9/11/2001 in support of the Global War on Terror or a recipient of the Freedom Alliance Scholarship Fund.


3. Candidates must be honorably discharged, retired, or medically discharged from the military.


4. Candidates must complete the application for consideration and provide all requested documentation.


Application for Consideration


Candidates must provide complete and honest responses to requests found in the Application for Consideration.


Proper documentation provided in support of the Application for Consideration includes those documents necessary to confirm military service, medical history, injuries, and family status and family history, including DD-214/DD-1300, disability rating from Veterans Administration, or other documents requested.


Among other things, completion of the Application for Consideration authorizes Freedom Alliance to complete a credit history, criminal background check, social media checks, or other background checks if needed.


Candidates will meet with member(s) of the “Driven to Serve” program committee to review the application and documentation if requested.  Candidates will be interviewed by Freedom Alliance, or its authorized representative, for consideration by the program.


Completion of the Application for Consideration does not guarantee acceptance to the “Driven to Serve” program.  Acceptance into the “Driven to Serve” program does not guarantee ownership through the program.  Acceptance merely means that the service member has been pre-screened and approved to enter the “Driven to Serve” program and be considered to receive a vehicle, should one become available and other criteria be met.


Candidates must provide copy of valid driver’s license and proof of insurance at time of award.


Terms of Transfer of Title


If selected to receive the full vehicle title, Candidate shall accept all legal and financial obligations associated with the vehicle.  Candidate shall continue to utilize vehicle as her or his primary transportation. The candidate will accept the vehicle “As is.”


Candidate will be responsible for all repairs, maintenance, upkeep, insurance, and other associated responsibilities.


Acknowledgement


I hereby acknowledge receipt of the foregoing Protocols relating to my participation in the “Driven to Serve” program. Furthermore, I agree to keep and use the vehicle for a period of three years before selling, transferring title, or gifting the vehicle to another party.


Submitting an application is consent to all terms and protocols.  I understand that by submitting this application, I consent to a check of my credit history and other background checks as determined necessary.  I agree to submit to an in-person or phone interview at Freedom Alliance’s sole discretion.  I also acknowledge that submission of an Application for Consideration does not guarantee selection for a vehicle.

To the fullest extent permitted by applicable law, I hereby irrevocably waive all legal and equitable rights relating to all liabilities, claims, demands, actions, suits, damages, and expenses, whether sounding in tort, contract, or any other legal theory, now known or hereafter known in any jurisdiction, arising directly or indirectly from my participation in or application to the “Driven to Serve” program (collectively, “Claims”).  I covenant not to make or bring any such Claims against Freedom Alliance, its legal representatives, partners, or assigns, and forever release and discharge Freedom Alliance and US Bank from liability under such Claims. I understand that I am not entitled to financial compensation from Freedom Alliance, partners, or any of its legal representatives or assigns as a result of submitting an application, being granted a vehicle title, or for my participation in the “Driven to Serve” program.


Custom Wheelchair Agreement

ELIGIBILITY: The following is a series of Protocols relating to the "Custom Wheelchair Agreement" program.

  • Candidates are service members who have deployed post 09/11/2001 in support of the Global War on Terror.
    • AND suffered a debilitating injury during combat.
      • Injury received during combat lead to other disabilities/wounds: amputation, immobility, spinal issues, etc.
    • OR suffered a debilitating injury while deployed, not necessarily in direct combat (e.g., fall from a tank or other vehicle).
  • Candidate suffered a debilitating injury while training for a combat deployment.
  • All others evaluated on a case-by-case basis.

Acknowledgement


I hereby acknowledge receipt of the foregoing Protocols relating to my participation in the "Custom Wheelchair" program. Furthermore, I agree to keep and use the wheelchair for a period of 3 years before selling, transferring title, or gifting the wheelchair to another party.

Offshore Experience Agreement

ELIGIBILITY:  Service members and veterans who have been wounded by enemy action post 09/11/2001 or serve in the special operations community and have multiple deployments are eligible to apply. Freedom Alliance selects groups of combat veterans to attend the Outdoor Adventures and the Offshore Experience Expeditions based on the information provided in the applications to put together groups that have commonalities – from deployment areas and times, similar burdens, and other factors to promote a quick group cohesion.

 

Submitting an application is consent to all terms and protocols.  I understand that by submitting this application, I agree to submit to an in-person or phone interview at Freedom Alliance’s sole discretion.  I also acknowledge that submission of an Application for Consideration does not guarantee selection.  I understand that by submitting this application, I agree to notify Freedom Alliance of any changes to the above-listed information.

War Heroes on Water Agreement

ELIGIBILITY:  Service members and veterans who have been wounded by enemy action post 09/11/2001 or serve in the special operations community and have multiple deployments are eligible to apply. Freedom Alliance selects groups of combat veterans to attend War Heroes on Water based on the information provided in the applications to put together groups that have commonalities – from deployment areas and times, similar burdens, and other factors to promote a quick group cohesion.

Freedom Alliance selects groups of combat veterans to attend the Outdoor Adventures and the Offshore Experience [Expeditions] based on the information provided in the applications to put together groups that have commonalities – from deployment areas and times, similar burdens, and other factors to promote a quick group cohesion.

ELIGIBILITY:  Service members or veterans who have been wounded by enemy action post 9/11/2001 or serve in the special operations community and have multiple deployments. Freedom Alliance selects groups of combat-veteran families to attend family vacation/retreats based on similar information provided in the application.


Submitting an application is consent to all terms and protocols.  I understand that by submitting this application, I agree to submit to an in-person or phone interview at Freedom Alliance’s sole discretion.  I also acknowledge that submission of an Application for Consideration does not guarantee selection.  I understand that by submitting this application, I agree to notify Freedom Alliance of any changes to the above-listed information.

Financial Assistance

ELIGIBILITY: Service members and veterans who have been wounded by enemy action post 9/11/2001 in support of the Global War on Terror and: 

  • are currently in treatment at a military hospital.
  • or are enrolled in an inpatient treatment program. 
  • or are seeking help through an AW2 Advocate / Wounded Warrior Battalion Liaison.


PLEASE NOTE: Freedom Alliance does NOT provide rent / mortgage assistance.

I hereby release, discharge and agree to save harmless Freedom Alliance, its legal representatives or assigns, and all persons functioning under its permission or authority, or those for whom he/she is functioning, from any liability by virtue of any use consistent with the purpose set forth herein.  For consideration that I acknowledge, I grant to Freedom Alliance to unlimited right to use my image, name, likeness, and information in connection with the program, which may be included in marketing materials (including print, websites, or other electronic forms of media made available nationwide).  In exchange for the intangible value I will gain by being featured in these materials, and other good and valuable consideration, I give Freedom Alliance permission for such and publicity for such purposes. 


To the fullest extent permitted by applicable law, I hereby irrevocably waive all legal and equitable rights relating to all liabilities, claims, demands, actions, suits, damages, and expenses, whether sounding in tort, contract, or any other legal theory, now known or hereafter known in any jurisdiction, arising directly or indirectly from my participation in or application to the program (collectively, “Claims”).  I covenant not to make or bring any such Claims against Freedom Alliance, its legal representatives, or assigns, and forever release and discharge Freedom Alliance from liability under such Claims. I understand that I am not entitled to financial compensation from Freedom Alliance or any of its legal representatives or assigns as a result of submitting an application or participation in the program.




DISCLAIMER and RELEASE: I release Freedom Alliance from any and all responsibility for any damages incurred in any activities or other expenditures upon which monies received from Freedom Alliance were used. I also acknowledge that by accepting a grant, Freedom Alliance has permission to use a photo, information and accounts of my situation, and the situation of my family members for its programs and publicity purposes. I will use the funds given to me only for the purposes stated in this application.
