By submitting this form, I agree to pay the musicians on the day of the performance in cash or checks payable to Midwest Young Artists.  A $100 deposit to MYAC is required when you return this form and we confirm that we can provide musicians .  

After we have looked at your expectations, we will contact you and determine if we can meet your needs with our student musicians.  At that time, you will will be asked to make a non-refundable deposit of $100, by check or credit card.  This deposit will be credited towards the payment owed musicians on the day of the gig.
