Current Milken students can use this form to:

Add a primary major or change their current SPH major to another SPH major ;
Add or change a concentration in their SPH major;
Add a second major or minor in SPH;
Add a second major in another school*; and
Add minors in another school*

*You are responsible for checking that you meet all necessary requirements to add a major or minor in a non-SPH school, meet with an advisor in that school, and ensure you have completed any necessary paperwork for that major or minor. We may require that you submit proof of approval to our office before declaring an academic program outside of GWSPH. Students outside of GWSPH interested in declaring a major, minor, or concentration at GWSPH should contact their home advising office for additional instructions. We will require a course progression plan before approving any declarations for GWSPH programs. If your home school advising office uses a paper form to declare programs outside of your home school, please use this link to upload your form for our review and approval. Major declarations are processed on the last working day of each month, excluding university holidays. In order to be processed on the final working day of the month, you must submit your declaration one calendar week prior. For example, if we process declarations on January 31, you must submit your declaration by the close of business on January 24. If you submit after January 24, your declaration will be processed on February 29, the last working day of the next month. The SPH Undergraduate Advising Office processes major, minor, and concentration declarations at the end of each month, but will not process declarations during November or April.

If you want Milken to be your home school, you must first submit an Internal Transfer Request form to the Registrar (found here). The Registrar will not process internal transfer requests during November or April.

The GPA requirements to declare a GWSPH major are:

Exercise Science: 2.50
Nutrition: 2.50
Health Data Science: 2.75
Public Health: 2.75

Please upload proof that you have spoken with the department hosting the academic program you are adding and that they approve of you joining their program. An email chain including you and a representative from the host department, a letter of approval from the program director, a signed major/minor declaration form from the program's host college, etc. are all acceptable documents.

Please upload an amended four-year plan that includes this change to your academic program. Please be sure to sequence the courses in the order provided in your program guide and to include the appropriate prerequisites. If your plan includes courses for other programs, please highlight the courses you intend to apply toward your GWSPH major/minor/concentration.

It is your responsibility to read and understand your new program's requirements. Failure to do so will result in a delay in your graduation. GWSPH will not make exceptions to the course progression plans included in each guide. An inaccurate submission will not forgo you of the requirements provided in your program guide.
