Thank you for your interest in applying for an HFLS Access Advance. Completing this application is the first step in the advance process.
Once you begin completing the application you will not be able to save your work and continue completing the application at a later date. Therefore, please make sure you have the following information handy (for you and for your spouse, if you are married):
- Annual pre-tax salary (if you are paid by the hour, please estimate your pay for an entire year at your hourly rate)
- Start date at current job (month and year)
- Social Security Number
- A copy of your photo ID (a scan of photo taken on your phone is fine, as long as it's readable)
Please also be prepared to provide a voided check or bank document that includes your bank name, checking account number, and routing number so HFLS can deposit your Access Advance directly into your checking account through electronic transfer.
After submitting your application you will receive an email requesting that you verify your signed response. You must click on the link in this email in order for HFLS to process your advance.
If your application is approved, you will receive the following documents by email within five business days.
Promissory Note - This is the salary advance agreement; it specifies the amount of the advance and the terms of repayment. Your signature indicates your agreement to repay the advance on these terms. If you are married, your spouse must also sign the promissory note.
Payroll Deduction Authorization - Instructs your employer to deduct funds from your wages every pay period until the full amount of the advance is repaid.
Authorization for Automatic Payments (ACH authorization) - Allows HFLS deposit your salary advance into your checking account by electronic transfer. Also authorizes HFLS to withdraw bi-weekly repayments directly from your bank account only if you leave your current employer and still have an outstanding balance to repay on your advance.
Be sure to check your email spam/junk folders for the email that contains the documents above. When HFLS receives these signed documents, we will deposit your advance funds into your checking account through direct deposit. (Please note that it may take 1-3 days for the funds to appear in your checking account.) The sooner you sign and return these forms, the sooner we can process your advance.
