This form is for requests for data clean-up in Salesforce. Clean-up requests can include:

• merge requests to clean up duplicate data, e. an account or contact in the database more than once
• data corrections/additions on fields containing incorrect/missing data
• contact information updates/requests to discontinue contact or change our contact level
• contacts with two names that need to be separated into two contacts in a single household, e.g. John and Joe Smith as one contact that needs to be split into two contacts in the Smith Household.

Note: Requests require that you supply the 18-digit Salesforce ID. You can easily grab the ID from the URL of the record. While on the record, look at the URL and copy the 18-digit code that is displayed after the object name, such as “contact” or “account,” e.g. in the following URL, the underlined section is the 18-digit Salesforce ID:  https://na59.lightning.force.com/lightning/r/Contact/003f4000007arLPAAY/view
