The purpose of this document is to create shared understandings about the ways in which the student’s authentic gender will be accounted for and supported at school. School staff, caregivers and the student should work together to complete this document. Ideally, each will spend time completing the various sections to the best of their ability and then come together to review sections and confirm shared agreements about using the plan. Please note that there is a separate document, the Gender Communication Plan for a student formally communicating information about their gender status at school.
The online Gender Support Plan allows you and your team to save and resume filling out the plan at any time. This allows you to share the plan among team members as needed. When you “save and resume” the form, you will be asked for an email and password. When those are entered, you will receive an email with a link to continue the form. The team leader can then forward that email and password to any team member, who can sign in (using the team leader’s email address) and continue filling out the form.
When the Gender Support Plan is finished, click "Review" and you will be directed to a page with all of your responses. From this page you may edit, download and/or print the completed plan. YOU DO NOT NEED TO CLICK "CONFIRM" TO SUBMIT YOUR PLAN. IF YOU DO CLICK CONFIRM; YOU CAN IGNORE THE MESSAGE YOU WILL RECEIVE WHEN YOU "REVIEW" THE PLAN. USE YOUR BROWSER'S BACK BUTTON TO GO BACK TO YOUR COMPLETED FORM.
