CAMVETS Participant Registration Form

Thank you for your interest in participating in the Fall 2019 CAMVETS program in Cleveland, OH!


We are looking for Makers and Documenters to comprise the Challenge Teams dedicated to building solutions to the unmet needs of their Veteran team member.

Please fill out this form to tell us about your expertise and interests so that we can try to place you on the appropriate team. We will contact you soon with this information and additional details.

Expertise and Interests








Project Leader


-Team creation: help to identify the expertise your team will need to create a solution. If you have experts in your network, recruit them to participate on your team. 


-Pre-event communication: lead team discussions on Slack to ask you Veteran team member questions and to start brainstorming ideas.


-Kick-off event: lead your team in defining a solution to the Veteran challenge and devising a plan to create a working prototype during the makeathon


-Pre-makeathon: work with your team to refine your plan and identify the materials and equipment that your team will need to build the solution. 


-Makeathon: lead your team in building the solution prototype and make sure they’re having fun!


-Closing Event: display and explain your team’s solution to industry leaders, public figures, and community members. 


-Post-event (optional): explore opportunities to work with our strategic partners to see how your team’s solution might move forward as a potential commercial product. 


Maker


-Pre-event communication: join your team on Slack to start discussing the challenge and to connect with your Veteran teammate


-Kick-off event: work with your team to define a solution to the Veteran challenge and devise a plan to create a working prototype during the makeathon 


-Pre-makeathon: work with your team to refine your plan and identify the materials and equipment that your team will need to build the solution. 


-Makeathon: work with your team to build the solution prototype and have some fun!


-Closing Event: display and explain your team’s solution to industry leaders, public figures, and community members.


-Post-event (optional): explore opportunities to work with our strategic partners to see how your team’s solution might move forward as a potential commercial product. 


Documenter/Communications


-Pre-event communication: join your team on Slack to start discussing the challenge and to connect with your Veteran teammate


-Kick-off event: work with your team to define a solution to the Veteran challenge and document the plan laid out to create a working prototype during the makeathon 


-Pre-makeathon: document the materials and equipment that your team will need to build the solution. 


-Makeathon: attend a brief training session and document the design process and features of the working prototype solution your team builds in the 3 days. Work with IP experts to draft provisional IP paperwork from your documentation. Serve as the communications liaison during the event to explain to organizers and observers what the team is working on. 


-Closing Event: Put together a presentation to display your team's solution to industry leaders and community members. 


Mentor


-Kick-off event: Help teams to first understand the Veteran challenge and second devise a solution strategy to address that challenge. Help to keep them on task. 


-Pre-makeathon: Be available to answer questions that teams have regarding their challenge and solution strategy (via Slack, email, or other preferred channel)


-Makeathon: Help teams to stay on task and troubleshoot any issues with their solution prototype. 


-Closing Event: Attend the closing event to see all of the solutions displayed and share the experience with community members. 


-Post-event (optional): guide teams in exploring opportunities to work with our strategic partners to see how their solutions might move forward as potential commercial products.  


Team Support


-Pre-event: Familiarize yourself with the physical/mental health conditions of the Veteran participants. 


-Kick-off event: Help teams to understand the Veteran challenge and to devise a solution strategy that will promote the mental and physical wellbeing of the Veteran. 


-Pre-makeathon: Be available to answer questions that teams have regarding their challenge and solution strategy (via Slack, email, or other preferred channel). 


-Makeathon: Help teams to make sure that their solution prototypes comply with your physical/mental health recommendations. 


-Closing Event: Attend the closing event to see all of the solutions displayed and share the experience with community members.

Media Consent and Release

Please read the following Consent and Release form. By accepting, you agree to let us use your information and images taken during the events in order to raise awareness about the CAMVETS program.

Effective as of the date shown below, approval for past use and permission for present and future use is being granted to Challenge America of PO Box 4467, Basalt, Colorado, 81621, to use quotes, pictures and/or voice recordings of you, the Recorded Party. By selecting “I accept” below, you are giving Permission as more fully explained in this Consent and Release. The Recorded Party is an adult and fully authorized to accept the terms of this Consent and Release. 

For a valuable consideration, receipt of which is hereby acknowledged, the Recorded Party hereby grants to Challenge America its agents, employees, licensees, and successors in interest (collectively, the "Released Party") all ownership rights and the absolute and irrevocable right and permission to copyright, use and publish the recorded image and/or voice of the Recorded Party (the "Image and/or Voice") that has been (or is being) obtained pursuant to this Consent and Release. 

The Image and/or Voice may be copyrighted, used and/or published individually or in conjunction with other photography, video works, and recordings, and in any medium (including without limitation, print publications, public broadcast, CD-ROM format) and for any lawful purpose, including without limitation, trade, exhibition, illustration, promotion, publicity, advertising and electronic publication. 

The Recorded Party represents and warrants that (i) no other party has been granted an exclusive license with respect to the Image and/or Voice, and (ii) no other party's authorization or consent is required with respect to the permission granted to the Released Party under this Consent and Release. 

The Recorded Party waives any right that the undersigned may have to inspect or approve the Released Party's use of the Image and/or Voice, or the advertising copy or printed matter that may be used in connection with the use and/or publication of the Image and/or Voice. The undersigned releases the Released Party (and all persons acting under its permission or authority) from all claims for libel, slander, invasion of privacy, infringement of copyright or right of publicity, or any other claim related to the Image and/or Voice (collectively, "Claims"). This release includes without limitation any Claims related to blurring, distortion, alteration, optical illusion, digital alteration, use in composite form, whether intentional or otherwise, or use of a fictitious name, that may occur or be produced in the processing or publication of the Image and/or Voice.
