iMentor Post-Secondary Program Emergency Fund Application
This fund helps recent iMentor high school graduates and current iMentor post-secondary students request and receive additional funding for personal situations. This application has three (3) parts.

There are pre-determined criteria, including filling out and submitting this application, that govern whether you will be eligible for emergency funds. If you meet these criteria, you will be awarded emergency funding.
Awards are made on a one-time only basis and are not renewable.
You are solely responsible for these funds, iMentor is not responsible for replacing any lost or stolen funds that were provided to you as part of the Emergency Fund.
You are responsible for any liabilities or obligations incurred in connection with the Emergency Fund, including, but not limited to, U.S. federal, state, or local income tax obligations.
