Undergraduate and New Members must speak with their current Chapter President and Vice President of Operations to discuss their intention to self-disaffiliate. Submissions from undergraduate or new members will not be accepted.

By agreeing to the terms below I acknowledge that I am officially disaffiliating from Delta Phi Epsilon Sorority. To complete the disaffiliation process I will provide my electronic signature on the next page of this document. I agree to contact my chapter president to turn in my sisterhood badge as well as my vice president of operations to pay off any outstanding balance. I understand that if an outstanding balance remains unpaid to the chapter, I may be sent to a collections agency.
