GOED - Rural Economic Development Incentive (REDI) Request for Funds

If you have completed your Rural Economic Development Incentive (REDI) requirements as outlined in the contract and are ready to receive your grant funds, please read the following instructions carefully before completing and uploading the required documents on the next page.  Grant recipients are limited to one grant fund request per contract.

 

When all fields are completed on Page 2, click the submit button. Your claim will be sent directly to the Governor's Office of Economic Development (GOED) Compliance Office for review.

  

INFORMATION & DOCUMENTATION YOU WILL NEED

 

In order to receive your REDI grant disbursement, you will need to submit the following documents to make a claim:

 

A.     Letter of Request - Written letter on company letterhead stating:


·         Completion of the contract requirements

·         Number of new full-time positions created 

·         Eligible employees annual wage

·         County or counties where the eligible employees are located

·         Formal request for the grant amount

·         Signature verifying the accuracy of your claim

      


B.     Full Employee Report and New Employee Report -  The Excel spread sheet templates (accessed on Page 2 of this form) contain three tabs--one for instructions and two tabs for the two reports.  Click on the first tab at the bottom of the screen and read the instructions. Click on each of the next two tabs to complete the forms in Excel, save for your records and upload on Page 2.  Information about the current full-time employee(s) will be compared against the baseline number of employees as stated in your contract.  The reports will show a full 12 months of company payroll activity prior to the date of the grant request and employee details including:

      

        ·         Name or ID number

      ·         Position title

      ·         Hire date

      ·         Termination date, if applicable 

        ·         Hours worked

        ·         Wages paid

      ·         Benefits paid, if applicable

      ·         County of employment


C.      You will choose one of the following two options on Page 2 of this Request for Funds:


1.    12-Month Post-Performance - If you are requesting the grant fund at 12 months from the last eligible employees hire date, please submit your two most recent DWS Form 33H Unemployment Insurance Quarterly Reports from the Department of Workforce Services identifying the new full-time employees and/or a copy of an executed professional employee agreement (if you do payroll through a third-party provider) as defined in Utah Code 31A-40-102 (15)

2.     6-Month Post-Performance - If you are requesting the grant fund at 6 months from the last eligible employees hire date, please submit: 


      a). Two most recent DWS Form 33H Unemployment Insurance Quarterly Reports from the Department of Workforce Services identifying the new full-time employees and/or a copy of an executed professional employee agreement (if you do payroll through a third-party provider) as defined in Utah Code 31A-40-102 (15)

       

      AND --

      


          b). Copies of the new eligible employee(s)' pay stubs at 2-month, 4-month, and 6-month pay periods.  The pay stubs must show the following information:


a.     Name of employee

b.     Dates of pay periods 

d.     Hours paid

e.     Wages paid

This incentive is awarded on a post-performance basis.

6 Months Annualized Full-time Employment Completed
This incentive is awarded on a post-performance basis where an annualized wage at 6 months meets 110% of the county average wage requirement.

Terms: All Rural Economic Development Incentive grant fund claims will be processed through the GOED Compliance Department and then through the GOED Finance Department.  Disbursement of grant funds is based on the terms of the contractual agreement the company entered into with GOED and the State of Utah. 

 

By checking the Agree box below, the Officer of the applying business hereby certifies to the State of Utah, acting through the Governor’s Office of Economic Development as Administrator of the Rural Economic Development Incentive grant program, the following:


  • The Grant Recipient is a sole proprietorship, partnership, association, joint venture, corporation, firm, trust, foundation, or other organization or entity used in carrying on a business
  • The Grant Recipient is not a business engaged in construction, staffing, retail trade or public utility activities
  • The Grant Recipient increased the number of full-time employees within the business from the time of application to the time of request by at least the number of positions as found in the Letter of Request and is paying the new eligible full-time positions 110% of the average wage of the county
