Positive behaviour support plan auditing tool.

Using the history, wishes, needs and preferences of the individual to update the positive behaviour support plan.
Measure of Success:
Auditing of behaviours that challenge looking at time of day, day of the week, staff present and any identifiable triggers.
Priority 3:
Train the staff team in providing a person centred support plan around activities and motivation; supporting the individual to live a meaningful and purposeful life and decrease behaviours that challenge.
Measure of Success:
Staff observations and supervision.
