Priority 1:
Incident reporting to be effectively used to review RPI use across the organisation, with a reporting and communication flowchart to be implemented. An online reporting system is in place, which ensures the effective communication of incidents, as well as accountability in the completion of actions. The reporting form is to be reviewed to ensure that the information gathered is in line with aiding the company in restraint reduction and supporting services when needed.
Measure of Success:
A report on the incidents recorded as using restraint is run on a weekly basis, should these incidents be outside of the expected activity for a service/person supported, further actions can be set. Monthly restraint reduction meetings will be carried out, with information on restraint statistics being reviewed. These reports will be able to form part of the annual training needs analysis for each service, to ensure that the least restrictive practices are in place.
Priority 2:
All incident or "near misses" should both lead to learning, this process is not about assigning blame, more it is about looking at an incident and asking whether this can be avoided in the future, or managed more effectively. Equally if the incident was well managed, what information can we share with the staff team. This review process must also improve quality of life, this being for both staff and the people supported.
Measure of Success:
Following on from Priority 1, once the incident reporting forms have been reviewed, these forms will encourage consistent post incident review and debrief processes. This process will be audited at service level by the Accomplish quality team. Feedback from which will be shared at the monthly restraint reduction meeting and actions set when required.
Priority 3:
People we support are to be involved in the delivery of physical intervention/Positive Behaviour Support training where possible. Co-production of training and materials with people support who have experience in the subject matter. This will help to enrich and enhance the training and provide a valuable role in reducing restraint used.
Measure of Success:
As part of the monthly restraint reduction meetings, trainers and operations will be asked to identify potential people supported who maybe interested in being involved in training sessions and/or restraint reduction as a whole. This will be explored initially, from a "case study" perspective. E.g. a person supported sharing their experiences in either written word or video, if they do not feel confident to attend the training/meet with staff in person. This will be explored as part of annual training and also new certification, as well as company induction for staff.
