There will be a meaningful reduction in the frequency of use of each restraint & an overall reduction in the restraints in use across the organization. In any instance where these is not a reduction in the use of a restraint there will be a review to assertion why leading to an action plan. The review will have multi disciplinary involvement where appropriate
Priority 2:
Our quality assurance (QA) policy states ‘We also believe that continuous improvement & learning …… is a key aspect of quality’. Our QA policy includes an annual personalization themed audit which will also be used for often if we feel that this can further improve quality. The goal is for this to be completed for every person we support within 12 months of us starting to provide a service.
Measure of Success:
Quality assurance audits provide feedback to the service, an action plan & are used by Amida management team to identify organization trends and to drive quality improvement at an organizational level. Each service also has a Service Improvement Plan which collates actions from all audits, complaints, compliments & any other relevant feedback. The Service Improvement Plan also sets out the services plans for the future. This part of the improvement plan should be creative & owned by the service user, for example a poster they have made supported by actions on the Service Improvement Plan. For single person services this may strongly reflect the support plan & annual review goals.
Priority 3:
All support plans are based on risk assessment, assessment of needs & use of person-centered planning tools to ensure personal needs, wishes, preferences & dreams are identified as well as needs. Support plans with be predominantly positive & proactive, they will describe people talents and how to help them use these abilities. They will also describe people dream and goals and how to help them achieve these
Measure of Success:
People we support will experience support which they & those close to them described as positive & proactive. Amida quality audits will show that support plans describe peoples talents, dreams & goals. The evidence of implementation of the support plans will show talents are used, working towards goals & dreams is part of everyday life.
