Priority 1:
To implement and support a network of in-house ‘champions’ of more highly trained staff to role model day-to-day best practice. For them to support or lead on reflection and learning from incidents and to assist in the creation and on-going review of person-centred restrictive intervention plans. To problem solve and seek to better understand the behaviours of our supported individuals and to offer structured practice to their colleagues where appropriate of both the theory and practical aspects of the formal training programmes, supporting trainers on courses where appropriate. To have a greater understanding and increased responsibility around record keeping and reporting of incidents of behaviours that challenge.
Measure of Success:
Staff identified and trained. They will then lead by example, role-modelling best practice for their colleagues and working with other teams where appropriate, helping to improve overall organisational communication and effect a reduction in restrictive practice in their service. This should be reflected by an overall reduction in restrictive practices being used across the organisation and an improved quality of life for our supported individuals.
Priority 2:
To improve and streamline the incident reporting systems currently in place to ensure higher quality recording and that incidents are reported more quickly to allow for multidisciplinary review and a timely response where appropriate. To ensure that incidents are reported in line with the Mental Health Units (Use of Force) Act 2018 and data is acted upon to ensure a reduction in restrictive approaches and practice.
Measure of Success:
A change of reporting system covering incidents of violence, aggression and behaviours that challenge which allows for the capture of quality data to enable the organisation to respond effectively and quickly in the review and implementation of any relevant control measures. Reports of incidents of behaviours that challenge to consistently be sufficiently detailed and contain quality data. The timeframe between incident and form submission will be reduced.
Priority 3:
To increase the quality and consistency of the debriefing process for both supported individuals and staff following an incident which involved the use of a restrictive physical intervention or other restrictive practices. Where a debrief with a supported individual is appropriate, this will enhance our understanding of the reasons for the incident or behaviour and enable us to improve or adapt the person’s PBS Support Plan with their input and support wherever possible. It will also facilitate the process of rebuilding relationships and supported individual input will help to inform this approach.
Measure of Success:
The design and implementation of a debriefing form, flow chart and training which is designed to support both our staff and the individuals we work with post-incident. Staff will receive training to help to understand the benefits of this approach and to ensure a consistent process. The thoughts and experiences of our supported individuals are crucial to our understanding of an incident and should result in a reduction of behaviours of concern as we get to better know each other.
