It involves using the best evidence you have about the most effective care of individuals, using it with the person’s best interests in mind, to the best of your ability and in such a way that it is clear to others that you are doing it.
Measure of Success:
Ongoing monitoring and reviewing, having a staff champion in each team that takes responsibility for the work that has been done. 6 monthly reviews. Additionally when all the clients we support are living lives that make sense to them, free from the restrictions of institutional living.
Priority 3:
Train the staff team in providing a person centred support plan around activities and motivation; supporting the individual to live a meaningful and purposeful life and decrease behaviours that challenge.
