Please note that many of the field trip options have been completely booked as of August 14, 2019. Only trips with availability are included on this form. If you are interested in a trip that is not included, please reach out to fieldtrips@act1az.org to be added to the waitlist.

Thank you for your interest in providing your students with an educational arts field trip this year! Please fill out this form as completely as possible to request a field trip. We will do our best to accommodate your first choice, however, in the event that a field trip is full we will work to accommodate one of your other choices. We will respond within 14 days regarding the status of your request. Your field trip is not confirmed until you receive a confirmation email from Act One.

Group Information
In an effort to more effectively and efficiently manage the field trip program and to better communicate with you regarding your field trip, we are asking that each request for a group be made individually. 
If you are registering more than one group for your school, you will have that opportunity by choosing the "Add Another Group Request" link on the bottom right. 




          
Field Trip Choices

Your group will receive one field trip; schools can request field trips for different groups of students, but we ask that the same student only go on one field trip per year. Act One will attempt to fill your Top Choice for a field trip. Because of demand, field trips fill up quickly, requiring us to look to one of your Back-up Choices. We recommend that you select up to three separate field trips to improve your chances of receiving a field trip. Act One will do our best to accommodate your group in the top choice you selected; however, if your top choice is full, we will then look at placing your group into one of your back-up options.

If you are not interested in any field trips beyond your top choice, please select N/A for your other choices.
