The Free Application for Federal Student Aid (FAFSA) is a form that can be prepared annually by current and prospective college students in the United States to determine eligibility for federal (governmental) student financial aid. You are eligible to complete the FAFSA if you are a: U.S. Citizen or U.S. National DACA Recipient, have a Green Card, have an Arrival-Departure Record, have a Battered Immigrant Status, have a T-Visa.

PARTICIPATION AND GPA POLICY:

 

All students in the Next Level Scholars program must commit to actively participating in all program activities, including: required surveys, in person events, virtual events, text/email communications, senior advising and school based meetings as required by the Next Level Scholars High School Adviser. Attendance for in-person and virtual events in the Next Level Scholars program will be counted by completing the survey and/or evaluation at the end of the experience.


All students must maintain a 2.0 cumulative G.P.A. at the end of the each semester to be eligible to continue with the Next Level Scholars program. In addition, if a student has a G.P.A. below 2.0 for the current semester at the time of an in-person trip, the student is not permitted to attend the event, resulting in an absence.


Sophomore and Senior students may not have more than one absence at Next Level Scholars events (virtual or in-person). Junior students may not have more than two absences at Next Level Scholars events (virtual or in-person). If a student exceeds the permitted absences, they may be dismissed from the program.

 

If a student is not in compliance with the Next Level Scholars Dress Code for in person events as determined by the high school adviser, they may not be permitted to attend the Next Level Scholars trip, resulting in an absence.


IN PERSON FIELD TRIP – HEALTH SCREENING & PROTOCOL AGREEMENT:


Prior to attending a DRDFS field trip, event or meeting, students and their parent(s) or guardian(s) may be required to complete a Health Screening checklist. If a student is feeling unwell, has any symptoms listed on the health screening checklist, or has been recently exposed to COVID or similar contagious illness, they will not be permitted to attend the field trip. In addition, masks or other preventive measures may be required to be worn on the bus/transportation or at venues or other trip locations based on local laws and policies.  Students must agree to follow this policy as well as any additional health screens/policies required by DRDFS, the event venue, or campuses we visit. Please note this policy is subject to change based on developing circumstances and new guidance as it becomes available, and that the safety of our students and other program stakeholders will always be our top priority.


If a student is unable to attend an in person DRDFS field trip, event or meeting, a virtual option will be made available so that the student can meet program participation requirements.

 

DRESS CODE:

 

Student in Next Level Scholars (A Program of Detroit Regional Dollars for Scholars) must:

 

  • Wear their Next Level Scholars provided t-shirt or sweatshirt at each event. No alterations are permitted.

  • Wear their Next Level Scholars provided face mask or personal face mask that meets the requirements listed below.

  • Wear clothing that provides adequate coverage of the body, and conceals undergarments at all times.  Students must NOT wear clothing that does not provide adequate coverage, such as see-through or fishnet fabrics, skirts or shorts that are shorter than mid-thigh in length (must extend beyond the fingertips when arms are fully extended), sagging pants, or tattered or torn clothing.

  • NOT wear clothing, accessories, or jewelry that contain writing, pictures, symbols or any other insignia that (i) are crude, vulgar, profane, obscene, libelous, or sexually suggestive, (ii) degrade any cultural, religious or ethnic values, (iii) advocate racial, ethnic, or religious prejudice or discrimination, or (iv) promote sex, the use of tobacco, drugs, alcohol, or any unlawful acts.

  • Wear shoes at all times.

  • Wear hats only when appropriate due to winter weather conditions. Non-winter hats of any kind are not permitted.

  • Wear headphones only while on buses.

  • Any exceptions or exclusions to this dress code will be at the discretion of the Next Level Scholars Program Director. Questions, including shirt replacement inquires should be directed to Marissa D'Angelo (mdangelo@drdfs.org).

STUDENT MEDIA RELEASE:


I understand that my photo and identifying information (i.e. name, grade level, and high school) may be released and used for the purposes of highlighting my achievements and promoting Detroit Regional Dollars for Scholars and its Dollars for Scholars Partner High Schools.

 

I understand that all pictures and recordings of my likeness belong to Detroit Regional Dollars for Scholars, and I will not receive payment or any other compensation in connection with them.

 

I understand and agree that Detroit Regional Dollars for Scholars and its authorized representatives shall have the exclusive rights, title, interest, including copyright, to any materials produced relating to the Dollars for Scholars Program, including those that feature my name and likeness.

 

I release and hold harmless Detroit Regional Dollars for Scholars, the Dollars for Scholars Partner Schools, and their respective representatives, employees, vendors, contractors, and officers from any and all actions, claims, damages, costs or expenses, including attorney’s fees, that may be brought against me or which relate to or arise out of any of my released information as specified above.

 Step 3: Parent/Guardian Permission for Medical Treatment

Whenever injury or emergency illness occurs to the student listed below while the student is under the supervision of a Next Level Scholars Adviser, every attempt will be made to notify the parent/guardian immediately. However, if a parent/guardian is not available and emergency treatment is necessary, the signature below by the parent/guardian authorizes the student to be transferred and treated in a timely fashion. The intention of this form is to grant authority to administer emergency treatment for any and all medical conditions.

 Step 4: Participation and Data Release Consent for Detroit Regional Dollars for Scholars

Purpose, Goals and Benefits:

As the parent/legal guardian of the minor student submitting this application, I consent for him/her to participate in all projects, events, and programs sponsored and/or organized by Detroit Regional Dollars for Scholars through the Next Level Scholars Program to gain support in preparing for life after high school. I understand that the student is responsible for his/her behavior, and will only perform volunteer work or participate in activities that he/she is comfortable doing. 


Services Offered:

Detroit Regional Dollars for Scholars programs provide students with the knowledge, resources, and experiences to help get them to the next level—post-secondary education.  Programming for 10th-12th grades includes SAT test preparation, etiquette training, leadership development, FAFSA completion workshops, campus visits, and more.

Detroit Regional Dollars for Scholars uses data to identify appropriate supports for each student and more effectively target programming to meet students’ individual needs. All data is kept strictly confidential and only used to help tailor services to the students served.


Data Requested for Release

Detroit Regional Dollars for Scholars may request access to the following data from your school and/or district: Academic Transcripts, PSAT 8/9, PSAT 10, PSAT NMSQT and/or SAT scores.


Protections:

Per the Protection of Pupil Rights Amendment (PPRA), you have the right to inspect protected information surveys and review copies of any materials used in your courses. To obtain copies of questions that may be asked of you or materials that may be used by or distributed to you, please contact Detroit Regional Dollars for Scholars directly.


Voluntary Participation and Release of Data:

You are not required to participate in Detroit Regional Dollars for Scholars programming and may stop or refuse services at any time. Likewise, you have the right not to consent to your participation and/or the release of your data. You will not face any penalties or lose any benefits to which you are entitled as a result of your responses on this consent form. Participation is completely voluntary and can be ended at any time.

The privacy and confidentiality of individual student information will be maintained throughout any use of the data, in accordance with school and/or district policy and applicable state and federal law, including, but not limited to, the Family Educational Rights and Privacy Act (FERPA). Data will be treated as confidential except as required by law.  


Disclosure of Information:

Pursuant to the Family Educational Rights and Privacy Act (FERPA) (20 U.S.C. § 1232g; 34 C.F.R. part 99), the written consent of a parent or eligible student is required before the education records of a student, or personally identifiable information contained therein, may be disclosed to a third party, unless an exception to this general requirement of written consent applies. If a student is age 18 years or older, or is enrolled in an institution of postsecondary education, he or she is an “eligible student” and must provide written consent for the disclosure of his or her education records or personally identifiable information contained therein.


This information may be disclosed to Detroit Regional Dollars for Scholars for the purpose of better tailoring the education experiences and support services that I receive.


I may withdraw my consent to allow the disclosure of this information at any time. If I do wish to withdraw my consent, I will submit my signed request for withdrawal in writing.

Como padre/tutor legal del estudiante menor de edad que presenta esta solicitud, doy mi consentimiento para que él/ella participe en todos los proyectos, eventos y programas patrocinados y/o organizados por Detroit Regional Dollars for Scholars a través del Programa Next Level Scholars para obtener apoyo en la preparación para la vida después de la preparatoria. Entiendo que el estudiante es responsable de su comportamiento, y sólo realizará trabajos voluntarios o participará en actividades con las que se sienta cómodo.

 

Servicios ofrecidos:

Los programas de Detroit Regional Dollars for Scholars proporcionan a los estudiantes los conocimientos, los recursos y las experiencias que les ayudarán a pasar al siguiente nivel: la educación post-preparatoria.  La programación para los grados 10 a 12 incluye la preparación para el examen SAT, entrenamiento de etiqueta, desarrollo de liderazgo, talleres para completar la FAFSA, visitas a los campus y más.

Viaje de Estudios Presencial - Acuerdo de Evaluación de Salud y Protocolo:

Antes de asistir a una excursión, evento o reunión del DRDFS, es posible que se requiera que los estudiantes y su(s) padre(s), madre(s) o tutor(es) completen una lista de Verificación de Salud. Si un estudiante se siente mal, presenta alguno de los síntomas enumerados en la lista de verificación de salud, o ha estado expuesto recientemente a COVID o a alguna enfermedad contagiosa similar, no se le permitirá asistir a la excursión. Además, es posible que se requiera el uso de mascarillas u otras medidas preventivas en el autobús/transporte o en las sedes u otros lugares de la excursión, según las leyes y políticas locales.  Los estudiantes deben estar de acuerdo en seguir esta política, así como cualquier otro examen de salud/política requerida por el DRDFS, el lugar del evento o los centros educativos que visitemos. Tenga en cuenta que esta política está sujeta a cambios en función de las circunstancias y de las nuevas indicaciones que vayan apareciendo, y que la seguridad de nuestros estudiantes y otros participantes en el programa será siempre nuestra máxima prioridad.


Si un estudiante no puede asistir a una excursión, evento o reunión del DRDFS en persona, se pondrá a su disposición una opción virtual para que pueda cumplir con los requisitos de participación del programa.

 الإذن الطبي لإخضاع الطالب للعلاج
متى حدثت إصابات أو أمراض طارئة للطالب المذكور اسمه أدناه أثناء خضوع الطالب لإشراف مستشار من برنامج "طلاب المستوى التالي"، يتم بذل كل الجهود الممكنة لإبلاغ ولي الأمر / الوصي على الطالب بشكل فوري. ومع ذلك إذا لم يكن ولي الأمر / الوصي على الطالب متاحًا وكان يلزم إخضاع الطالب للعلاج الطارئ، فإن التوقيع الموجود أدناه لولي الأمر / الوصي على الطالب يمنح التصريح بنقل الطالب وعلاجه في الوقت المناسب. إن الهدف من هذا النموذج منح السلطة لتنفيذ العلاج في حالات الطوارئ لكل الحالات الطبية بل وأي حالة طبية تظهر.

الموافقة على المشاركة وإصدار البيانات لبرنامج DETROIT REGIONAL DOLLARS FOR SCHOLARS

الغرض والأهداف والمزايا:

بصفتي الوالد/ الوصي القانوني للطالب القاصر الذي يقدم هذا الطلب، فإنني أوافق على مشاركته في جميع المشاريع والأحداث والبرامج التي ترعاها و/أو تنظمها Detroit Regional Dollars for Scholars من خلال برنامج Next Level Scholars (طلاب المستوى التالي) للحصول على الدعم بغرض الاستعداد للانخراط في حياة بعد المدرسة الثانوية. وأنا أدرك أن الطالب مسؤول عن سلوكه، وأنه لن يشارك إلا في الأعمال التطوعية أو الأنشطة التي يشعر بالراحة في القيام بها. 

 

الخدمات المقدمة:

تزود برامج Detroit Regional Dollars for Scholars الطلاب بالمعرفة والموارد والخبرات للمساعدة في نقلهم إلى المستوى التالي - وهو التعليم ما بعد الثانوي. وتتضمن البرامج المخصصة للصفوف من العاشر إلى الثاني عشر الاستعداد لاجتياز اختبار SAT، والتدريب على آداب السلوك، وتطوير مهارات القيادة، وورش عمل إكمال نموذج FAFSA، وزيارات الحرم الجامعي، وغير ذلك الكثير.


الرحلة الميدانية الشخصية - اتفاقية الفحص الصحي والبروتوكول المتبع:


قبل حضور رحلة ميدانية أو فعالية أو اجتماع تابع لبرنامج DRDFS، قد يُطلب من الطلاب وولي /أولياء أمورهم أو الوصي/ الأوصياء إكمال قائمة مراجعة الفحص الصحي. إذا كان الطالب يشعر بتوعك، أو ظهرت عليه أي أعراض مدرجة في قائمة مراجعة الفحص الصحي، أو تعرض مؤخرًا للإصابة بفيروس كورونا المستجد أو مرض معدٍ مشابه، فلن يُسمح له بحضور الرحلة الميدانية. بالإضافة إلى ذلك، قد يلزم ارتداء الأقنعة أو الخضوع للتدابير الوقائية الأخرى في الحافلة/ وسيلة النقل أو في الأماكن أو مواقع الرحلات الأخرى بناءً على القوانين والسياسات المحلية.  

يجب أن يوافق الطلاب على اتباع هذه السياسة بالإضافة إلى أي فحوصات/ سياسات صحية إضافية يتطلبها برنامج DRDFS أو مكان الفعالية أو الجامعات التي نزورها. يُرجى ملاحظة أن هذه السياسة عرضة للتغيير وفقًا لتطور الظروف والإرشادات الجديدة عند توفرها، وأن سلامة طلابنا وأصحاب المصلحة الآخرين في البرنامج ستكون دائمًا على رأس أولوياتنا.



إذا لم يكن الطالب قادرًا على المشاركة في أي رحلة ميدانية أو حدث أو اجتماع تعقده DRDFS، فسيتم توفير خيار افتراضي حتى يتمكن الطالب من استيفاء متطلبات المشاركة في البرنامج.
