I (named above) hereby grant permission to Musicopia to take photos and video (including but not limited to (1) live-streamed videos; and (2) photos and live or recorded video from Zoom or other online platforms) of the above named child’s participation in our programming and allow Musicopia and its partner organizations to use these photos and videos as they determine.  

I also grant permission to Musicopia to use photos and videos of the above named child that have been submitted by myself or my child and allow Musicopia and its partner organizations to use these photos and videos as they determine. 

Musicopia partner organizations include, but are not limited to, media, non-profit organizations, and public and private foundations.

I attest that I am authorized as a legal parent or guardian of above named child to sign this form. I acknowledge this release is firm and final and understand that Musicopia may proceed in reliance thereon.  I release any and all claims against Musicopia arising in connection with the use of my child’s image, likeness and voice.  I desire to assist the work of inspiring students towards a lifelong involvement in music and the arts by making the image, likeness and voice of the undersigned available.
