CSJ Awards 2021 - Application Form
Welcome to the application form for the CSJ Awards 2021.

We hope you find this form straightforward to complete but if you have any difficulties please do email us (CSJAwards@centreforsocialjustice.org.uk) and we will look to get back to you as soon as possible.

The application is divided into several parts across three pages, each asking about a particular aspect of your organisation.  You can navigate through the pages at any time to see all the different questions.  We recommend you take time, away from the form, to think through and plan your answers to the longer questions and then copy and paste these back in.  This will also mean you have a backup in case there is problem part way through you completing the form.

All questions marked with a red asterisk must be completed before submitting the form. Word limits are clearly indicated next to a question when applicable.

You are able to save the form and come back to it at a later date.  You can do this by clicking on the ‘Save my progress and resume later’ found at the top and bottom of each page.  You will be asked to enter an email and password.  Once complete, an email will be sent through to you with a link to the partially completed form.  You will need to enter the password to access this again.  Any problems, please contact the CSJ - CSJAwards@centreforsocialjustice.org.uk

Once a form is submitted it will be sent through to the CSJ and can no longer be amended.

Closing date: 11th October 2020

Through case study example, share how you helped an individual or group and the impact it had on them. Use the 4 parts to guide your answer:

What is the specific problem or negative situation the individual or group found themselves in that led to them receiving your help

Describe what you did and over what time period

What was the outcome of your intervention. Highlight the impact you have had and how this is being sustained
