WHAT THIS FORM DOES:
This form collects event registrations for any Program of which the Program ID is plugged into this form (see section "BEFORE SENDING THIS FORM OUT" on how to plug this in). In Salesforce, this means that upon submission,

1. The individual filling out this form will be found and matched with their existing record in Salesforce via their email address OR exact first name + last name match (OR newly created, if not found in the system);
2. The organization entered here will be matched in Salesforce (OR newly created, if not found in the system);
3. An affiliation record between the submitter and their organization will be updated (OR newly created, if not found in the system);
4. The individual will be added to the intended Program, with a Status of "RSVP Yes".
