SEP Application

The Summer Enrichment Program (SEP) serves young men who have finished the seventh grade and who are interested in enriching their education.

It will run from June 9 - June 30. During the program, students will be enriched academically, culturally, socially and physically.

The day begins at 8 a.m. and ends at 1 p.m. Participants are responsible for their own transportation to and from Saint Ignatius High School. Registration is $375. Two t-shirts and daily breakfast and lunch are included with the cost.

In the event reasonable attempts to contact me have been unsuccessful, I hereby give my consent for (1) the administration of any treatment deemed necessary by above-named doctor, or, in the event the designated preferred practitioner is not available, by another licensed physician or dentist: and (2) the transfer of the child to any hospital reasonably accessible.

This authorization does not cover major surgery unless the medical opinions of two other licensed physicians or dentists, concurring in the necessity of such surgery, are obtained prior to the performance of such surgery.
