Media Release

The media release form gives Shatterproof permission to use, reproduce, electronically publish, and display in all media a volunteer's name, photograph, and any information provided to Shatterproof by the volunteer. To protect the organization, Shatterproof will obtain a signed media release form from all volunteers.

Terms and Conditions

I hereby agree that all rights in and to any content submitted, are and shall remain the sole property of Shatterproof, free and clear from any claims by me or anyone acting on my behalf. Shatterproof’s rights include, but are not limited to, the rights, in perpetuity, to: Use, re-use, publish, and re-publish the Content; Alter, modify or otherwise change the Content in any manner Shatterproof desires; Combine the Content with textual matter and/or with other pictures and/or media; and, Use the Content for illustration, promotion, art, editorial, advertising, trade, publishing, or any other purpose whatsoever. I hereby release, discharge, and agree to hold harmless Shatterproof, Shatterproof’s legal representatives and assigns, and all persons acting under Shatterproof’s authority or those for whom he/she is acting, from any liability by virtue of any use of the Content or any changes or alterations made thereto. I hereby warrant that I am of full legal age and have the right to contract in my own name.

Volunteer Agreement

This Shatterproof Junior Board volunteer agreement demonstrates how we value our volunteers. We want to assure you that we appreciate your contribution to our organization. We are dedicated to ensuring that you have a quality volunteer experience that is both productive and rewarding.

Shatterproof commits to the following:

To provide adequate information, resources, and training so you may meet the expectations as described in your volunteer role description and guidelines.
To explain what is required of you and to support and provide encouragement to help you achieve the desired results.
To assign you with a named supervisor who will provide you with regular support and act as a ‘go to’ person.
To treat you with respect and courtesy at all times.
To be receptive to any comments and feedback from all volunteers.

The Shatterproof Junior Board member commits to the following:

To fulfill my role as outlined in specific project guidelines.
To share and receive approval for all speaking engagements.
To be proactive in my role in a way that furthers the mission of Shatterproof and raises awareness of it’s programs and advocacy opportunities.
To meet time and task commitments and to provide sufficient notice when not available.
To act in a manner that aligns with the objectives of the organization and that enhances the work of the organization.
