2021 Student Application


Welcome and thank you for your interest in the 2021 College Horizons (remote) summer workshop for Native American, Alaska Native and Native Hawaiian high school students!
This form is for NEW student applicants only. Please complete all pages with correct and updated information. We know it has been a challenging year and we hope to paint a full picture of your academic, family, and home life in order to evaluate how we may be able to support you in the college and financial aid process. 

The tentative program dates are July 9 - July 16, 2021. The daily program schedule will likely be 8:30am-12:00pm (small group meetings), 2-4pm (all group webinar or individual meetings with list maker, essay specialist, advisor), 7-9pm (small group study hall & office hours). Leading up to the program, we will also have you schedule individual meetings with your list maker, essay specialist, and a few small group introduction sessions so that you can make the most of your time together in July.

To eligible for our program, each student must:

  • Be American Indian, Alaska Native, or Native Hawaiian (You will need to provide a copy of relevant state/federal tribal enrollment documentation or from the Ho’oulu Hawaiian Data Center or Office of Hawaiian Affairs – we do not accept birth certificates of a student, parent, or grandparent as proof of enrollment).
  • Be a current sophomore or junior in high school.
  • Have a minimum 3.00 cumulative GPA (in core academic courses, not electives).
  • Be a citizen or permanent resident of the United States.
  • Submit a completed application packet by the deadline.

Technology

Since College Horizons will run our program remotely this summer, having access to a computer (laptop, desktop or tablet), a cell phone, and high speed internet are necessary. Many students have been issued school-based devices and internet hotspots. Consider what technology you will have access to in June and July, especially if you have to return school based devices. We need to have a clear understanding of your access to technology in your household.









TIME ZONES

College Horizons students span 6 time zones from Hawaii to the East Coast. Given that our 2021 programs will be held remotely, we will assign students to small groups based on time zones.

Counselor and Teacher Recommendations

Counselor Information (Required)
In lieu of a traditional Counselor Recommendation Letter, we ask that applicants have their counselor fill out our 2021 Counselor Form. Please indicate the name and contact information for the counselor who will be submitting a form on your behalf.

It is the student's responsibility to give your Counselor the link to the required Counselor Form. We only use the contact information below for verification purposes.

NOTE: The 2021 Counselor Form will ask simple questions on how your school offered instruction during the pandemic (in-person, hybrid, remote), the grading system (pass/fail, grades, etc.), if any courses of study changed (were honors, AP classes offered or decreased due to the pandemic). We will also ask Counselors to upload a copy of your current transcript.





We would also like you (the student applicant) to answer a few questions about your school counseling experience. The answers to these questions will not be shared with your Counselor. They help us have a better understanding of your situation.




Teacher Recommendation (Optional)
Teacher Recommendations are NOT required as part of your application. However, if you have a strong relationship with a teacher this year, you are welcome to ask them to submit a recommendation letter at this link to College Horizons on your behalf.

You (the student) are responsible for providing your teacher the link to the Teacher Recommendation Form, should you choose to submit one. We only use the contact information below for verification purposes.

Personal Essay

The CH team wants to be supportive and sensitive to students and families as you may choose to disclose the impact COVID-19 has had on you, your families and communities. We are at the one year mark of living through the pandemic. While we have shared in collective suffering, not all of us are moving through the pandemic in the same way. For example, some are experiencing illness, while some are healthy; some have lost relatives to the virus; some of our parents are unemployed and some of our parents kept their jobs; some of us feel worried or isolated and some of us feel excited or grounded. Seeing beyond our individual experiences and understanding the range of experiences of others helps our awareness and reminds us to be gentle and compassionate with ourselves and others.

Please know that it is your choice whether or not to write about your COVID-19 experience in your college application; this self-reflection is only for the purposes of your College Horizons application.
