Pets As Therapy only accept applications from UK addresses.
I consent to Pets As Therapy using photographs of myself, which may be taking during the time I am registered with Pets As Therapy. These would be used to help promote the beneficial work of the Charity and would potentially appear in: • Newsletters, magazines, newspapers, websites, promotional literature, DVDs, etc. I am aware that ALL publicity MUST be authorised in advance by a member of Pets As Therapy staff.

If you have two or more pets, then you can add these later. Please register your first pet now and then follow the instructions in our Volunteer Community.
(A dog that has NOT been immunised cannot be accepted as a Pets As Therapy dog)
DECLARATIONS

Registered Volunteer & Pets:

I confirm that once my pet is registered and I become a Pets As Therapy volunteer I will:

  • Abide by the  Volunteer Handbook and policies of Pets As Therapy
  • Abide by the policies and procedures of the establishments I visit with my pet
  • Maintain and present my Pets As Therapy pet in good health, free from parasites and well groomed
  • Not feed my Pets As Therapy pet a raw meat diet or one containing unpasteurised milk
  • Accept complete responsibility for my actions and my pet’s actions whilst on visits
  • Display my Pets As Therapy photo identity badge and my pet’s photo ID on all visits
  • Keep my PAT pet on a regular lead during visits.  (Not a training lead, flexi lead, retractable lead, head collar, check chain or restraining harness.)

Data Protection and Privacy:

I consent to Pets As Therapy processing my personal data for the purposes of processing my application and subsequent volunteer administration. I further consent to Pets As Therapy sending me information relevant to volunteering for the charity, to the establishments I visit and about the wider work of Pets As Therapy.

I understand that my contact details (only) will be passed to a Pets As Therapy Area Co-ordinator to facilitate registration and co-ordinate and support me in volunteering.

 

Pets As Therapy takes all reasonable care to ensure the security of my data in storage and processing.

Pets As Therapy does not sell or disclose my details to any third party individual or organisation unless required to do so by law. 

Driver Declaration:

If I drive a vehicle when acting as a Pets As Therapy volunteer, I hold a full driving licence, my vehicle has a current MOT, I have valid insurance and I have advised my insurers of (a) any driving convictions or health conditions that would affect my driving and (b) that I am undertaking voluntary work.   
GIFT AID
I want to Gift Aid my donation and any donations I make in the future or have made in the past 4 years, to Pets As Therapy. I am a UK taxpayer and understand that if I pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all of my donations in any tax year it is my responsibility to pay any difference. Pets As Therapy will claim 25p on every £1 donated.
